Carvedilol


Generic Name: Carvedilol (kar-VE-dil-ole)
Brand Name: Coreg

Carvedilol is used for:

Treating high blood pressure and mild to severe heart failure. Carvedilol may be used alone or with other medications. It may also be used for other conditions as determined by your doctor, which may not be listed in the professional package insert.

Carvedilol is a beta-blocker with alpha-blocking activity. It works by dilating (widening) blood vessels, slowing down the heart, and reducing blood pressure, which helps the heart pump blood more efficiently and reduces its workload.

Do NOT use Carvedilol if:

  • you are allergic to any ingredient in Carvedilol
  • you have asthma, a very slow heartbeat, very low blood pressure, heart block, or severe liver disease

Contact your doctor or health care provider right away if any of these apply to you.

Before using Carvedilol :

Some medical conditions may interact with Carvedilol . Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:

  • if you are pregnant, plan to become pregnant, or are breast-feeding
  • if you are taking any prescription or nonprescription medicine, herbal preparation, or dietary supplement
  • if you have allergies to medicines, foods, or other substances
  • if you have an adrenal gland tumor, diabetes, low blood sugar, liver problems, overactive thyroid, anaphylaxis, chronic bronchitis, chronic obstructive pulmonary disease (COPD), emphysema, or other lung disease
  • if you have heart failure; Prinzmetal angina (chest pain due to coronary spasm); slow heartbeat; narrowing of blood vessels of the legs, arms, stomach, or kidneys; or kidney problems
  • if you are scheduled to have surgery or receive anesthesia

Some MEDICINES MAY INTERACT with Carvedilol . Tell your health care provider if you are taking any other medicines, especially any of the following:

  • Catecholamine-depleting agents (eg, reserpine or monoamine oxidase [MAO] inhibitors [eg, phenelzine]) because lightheadedness when rising from a seated position and very slow heartbeat may occur
  • Certain medicines that act on the liver because the actions and side effects of Carvedilol may be increased or decreased
  • Anesthetic agents (eg, cyclopropane, trichloroethylene), antiarrhythmics (eg, amiodarone, quinidine, lidocaine, flecainide, propafenone), birth control pills, bupivacaine, calcium blockers (eg, diltiazem, verapamil), digoxin, ketanserin, mefloquine, phenothiazines (eg, chlorpromazine), or thioamines (eg, methimazole) because the risk of side effects on the heart may be increased
  • Alpha blockers (eg, prazosin) or hydralazine because side effects of lightheadedness when rising from a seated position may be increased by Carvedilol
  • Cyclosporine, insulin, narcotics (eg, codeine, morphine), or oral hypoglycemics (eg, metformin, glyburide, repaglinide) because the actions and side effects may be increased by Carvedilol

This may not be a complete list of all interactions that may occur. Ask your health care provider if Carvedilol may interact with other medicines that you take. Check with your health care provider before you start, stop, or change the dose of any medicine.

How to use Carvedilol :

Use Carvedilol as directed by your doctor. Check the label on the medicine for exact dosing instructions.

  • Take Carvedilol with food.
  • Taking Carvedilol at the same time(s) each day will help you remember to take it.
  • Continue to use Carvedilol even if you feel well. Do not miss any doses.
  • If you miss a dose of Carvedilol , take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once.

Ask your health care provider any questions you may have about how to use Carvedilol .

Important safety information:

  • Carvedilol may cause dizziness and lightheadedness. Do not drive, operate machinery, or do anything else that could be dangerous until you know how you react to Carvedilol . Using Carvedilol alone, with certain other medicines, or with alcohol may lessen your ability to drive or to perform other potentially dangerous tasks.
  • Carvedilol may cause dizziness, lightheadedness, or fainting. Alcohol, hot weather, exercise, and fever can increase these effects. To prevent them, sit up or stand slowly, especially in the morning. Also, sit or lie down at the first sign of dizziness, lightheadedness, or weakness.
  • WARNING: If you have angina or have had heart problems, do NOT suddenly stop taking Carvedilol without first consulting with your doctor. Severe chest pain, heart attack, very fast heartbeat, and irregular heartbeat may occur. If your doctor decides you should no longer take Carvedilol , the dosage should be gradually reduced according to your doctor"s instructions.
  • Heart failure patients - Talk with your doctor if you experience weight gain or increasing shortness of breath.
  • Diabetes patients - Carvedilol may mask signs of low blood sugar, such as a rapid heartbeat. Other symptoms, such as sweating, may still occur. Carvedilol may affect blood sugar levels. Your doctor may need to change the dose of diabetes medicine you take. Check blood glucose levels regularly.
  • Patients being treated for high blood pressure often feel tired and run down for a few weeks after beginning therapy. Continue taking your medication even though you may not feel quite "normal." Contact your doctor or pharmacist about any new symptoms.
  • If your doctor has instructed you to check your blood pressure and heart regularly, be sure to do so. Ask your pharmacist for help if you are unsure how to accurately measure your blood pressure or heart rate.
  • Carvedilol may cause decreased tearing (dry eyes) in patients who wear contact lenses.
  • Before having any surgical or dental procedures or receiving emergency treatment, tell the doctor or dentist in charge that you are receiving Carvedilol .
  • Additional monitoring of your dose or condition may be necessary if you are also taking decongestants, hydralazine, rifampin, medicines for asthma, diabetes, or thyroid conditions.
  • LAB TESTS, including blood pressure checks, may be performed to monitor your progress or to check for side effects. Be sure to keep all doctor and lab appointments.
  • Use Carvedilol with caution in the ELDERLY because they may be more sensitive to its effects, especially dizziness.
  • Carvedilol is not recommended for use in CHILDREN younger than 18 years of age. Safety and effectiveness in this age group have not been confirmed.
  • PREGNANCY and BREAST-FEEDING: If you plan on becoming pregnant, discuss with your doctor the benefits and risks of using Carvedilol during pregnancy. It is unknown if Carvedilol is excreted in breast milk. Do not breast-feed while taking Carvedilol .

Possible side effects of Carvedilol :

All medicines may cause side effects, but many people have no, or minor, side effects. Check with your doctor if any of these most COMMON side effects persist or become bothersome:

Change in vision; diarrhea; dizziness; drowsiness or trouble sleeping; dry eyes (especially if you wear contact lenses); lightheadedness; nausea; slow heartbeat; swelling of the legs; unusual tiredness or weakness; weight gain.

Seek medical attention right away if any of these SEVERE side effects occur:

Severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); bluish discoloration of hands or feet; chest pain; dark urine; fainting; flu-like symptoms; increased thirst; increased urination; loss of appetite; pounding in the chest; severe dizziness; slow heartbeat; swelling; tenderness of the upper right part of the stomach; unusual bruising or bleeding; yellowing of the skin or eyes.

If OVERDOSE is suspected:

Contact 1-800-222-1222 (the American Association of Poison Control Centers), your local poison control center (http://www.aapcc.org/findyour.htm), or emergency room immediately. Symptoms may include difficulty breathing, especially when lying down; fainting; lack of heartbeat; lightheadedness, especially upon standing; seizures (convulsions); severe dizziness; shortness of breath; unconsciousness; very slow heart rate; vomiting; weakness; wheezing.

Proper storage of Carvedilol :

Store Carvedilol below 86 degrees F (30 degrees C), away from heat, moisture, and light. Do not store in the bathroom. Keep Carvedilol out of the reach of children and away from pets.
